When do I need to arrive for my appointment?

Please arrive by the time that was given to you when the appointment was made.

Is there any paperwork I need to fill out?

     You can print the patient forms from this site and fill them out. 
     This will save you time when you come in for your appointment.

What do I need to bring with me?

     You need to bring your Driver's License, Insurance Card, Co-payment and
          Patient Forms.

Can I bring my children with me to the appointment?

     We ask that you do not bring your children with you for the appointment.
     We are unable to watch your children for you.
     You will not be able to watch your children during your examination, testing or procedures.
     The examination room are really small.

Is there anything special I need to do for my appointment?
     Relax, get your regular sleep, eat your regular food.
     Take your medications as prescribed.
     Wash, shower or bathe before coming to your appointment.
     Please do not use perfumes, skin or body lotions which may cause interference
          with the tests and procedures.

What should I wear?

     If your neck, upper back, shoulders, arms or hands are to be evaluated, tested
          or injected;
          Please wear a loose fitting short sleeved shirt or blouse. 
          Women can wear a Sports Bra or Swim Suit Top if they are having injections performed.

     Women, please wear a bra.

     If you low back, legs, or feet are to be evaluated, tested or injected;
          please wear loose fitting shorts.  Don't wear a dress or a skirt.

     Women, please not not wear pantyhose or long stockings.

     Please wear underwear.

Do I need to bring a driver?

     You may only need to bring a driver if you are having injections done.
     You will be told if you will need to bring a driver with you for your next appointment.
